Additional Forms
Once you have mailed in an application with your enrollment deposit you will receive a confirmation letter along with additional forms from Wildwood's registrar. These forms will need to be completed and brought with you to registration on the first day of your students session. If you need additional copies of anything please contact us and we'll promptly get you what you need.
Health Form
Since you're student will be spending a week away from you while at Wildwood its important that we be able to care for them in the best possible way. In order to do so we require information regarding your child and their medical background for their own safety and the safety of others. The information we require is standard and helps us better ensure a safe environment for everyone in attendance. Please be sure to complete this form and bring it with you when you register your child on their first day.
*Regarding Medication - If your child is currently take any medication (prescribed or over the counter) it must be noted on their health form and turned into our medical staff to be administered. We can not allow any medication to be kept in camper quarters for the safety of your child and others we must follow this rule. Please be sure to note any food or drug allergies so that we can keep your child safe.

In the event of a medical emergency we require authorization from you to transport your child off site for treatment. In addition, should relatives or family friends come to visit during our session we will need permission in writing from you to allow them to take your student off-campus for any reason. Please remember to bring this form with you when you register your child on their first day.
